Hospice Care
To meet the needs of our terminally ill nursing home residents, St Ann’s partners with Licensed and Certified Hospice Agencies and their staff to provide care that assures comfort and dignity at the end of life.
Families, residents and patients throughout our Communities have the right to choose whichever Hospice Agency they want to provide their care. St. Ann’s has relationships with both Visiting Nurse Service Hospice and Lifetime Hospice. Both agencies provide “routine hospice home care” to people in the their homes, be that senior retirement apartment, assisted living or skilled nursing facilities.
St. Ann’s has a special partnership with Visiting Nurse Service Hospice to provide short term pain and symptom management and end of life care in our Palliative Center for Caring, a dedicated five bed unit in The Heritage.
Most patient stays are for 3 to 7 days and if not for this special program, these patients would otherwise require a hospital admission. Every effort is made to create an ambiance of warmth, and caring, all in keeping with helping patients complete their lives with dignity, meaning and peace. VNS Hospice and St. Ann’s specially trained clinical staff are highly skilled at providing palliative (comfort-oriented) care.
