Our doctors, nurses and other specialists work with you and your family to ensure
a smooth transition from hospital to discharge. By using the latest equipment, the
newest practices and a team approach, we are helping many patients enjoy amazing

Expert Care with a Personal Touch
Our Transitional Care Program provides expert care to patients who are recovering
from a wide range of challenges.
- Rehabilitation for adults recovering from cardiovascular surgery, amputations, fractures,
hip or knee surgery, strokes, swallowing impairments, motor vehicle accidents, neurological
and cardiovascular diseases or other acute, complex medical conditions.
- Physical, occupational and speech therapy utilizing the latest equipment, newest
practices and a team approach.
- Clinical assistance with reaching your rehabilitation goals and developing a safe
discharge plan.
Our Clinical Team
Our staff is on-site full-time and has received specialized training in many state-of-the-art techniques:
- NDT stroke care allowing our therapists to maximize the neurological and functional recovery of our stroke patients.
- Fiberoptic Endoscopic Evaluation of Swallowing (FEES) allows our speech pathologists to provide immediate, on-site assessment of swallowing as well as recommendations for the safest eating/feeding plan.
- Specialized wound care certification to speed healing and reduce complications.
Sharon Osborne is the Director of Rehabilitation Services at St. Ann 's Community. She has a B.S. in Physical Therapy from Daemen College, an M.S. in Health Systems Administration from Rochester Institute of Technology, and certifications in Manual Lymph Drainage, Decongestive Physical Therapy, and Neurodevelopmental Training (NDT).
