Adult Day Services

St. Ann's Adult Day Services are comprised of two different services, Home Connection and Home and Heart.

Home Connection

Home Connection is a medical adult day service that provides quality care to individuals in a comfortable, stimulating, home-like environment. St. Ann's social workers develop personalized health care and recreation services plans for each person's health care needs, personal goals and requirements based on meetings with customers, their family caregivers and physicians' orders.

Health care services available:

• Case management and social work counseling
• Health care monitoring and skilled nursing care
• Physical, occupational and speech therapy
• Hygiene, bathing and personal care
• Nutrition and dietary counseling
• Podiatry visits
• Dental hygiene
• Hearing evaluations
• Lab and diagnostic studies
• Mental health referrals
• Transportation
• Priority access to St. Ann’s Community Rehabilitation and Skilled Nursing

Recreational services include:

• Recreation therapy: music, arts & crafts, gardening and more
• Field trips and outings to the mall, museums, theater, etc.
• Visits from area school children
• Group Exercise and individualized weight training and walking programs
• Religious and educational activities
• Trips to the hair salon

The Home Connection serves a large area of the community at no extra charge. Plus, we can help arrange transportation for medical appointments.

Home Connection is located at 1500 Portland Avenue. Hours of Operation are Monday through Friday, 7:00 a.m. - 5:00 p.m. Call 697-6361 for admission information.

Home & Heart

With several locations in the greater Rochester area, Home & Heart is a social adult day service helping to meet individual needs and personal goals by providing support services and social activities in a relaxed, home-like setting. Home & Heart is ideal for older adults and persons with disabilities living in the Greater Rochester area who may have diminished physical or psychological capacity, chronic illness, memory problems, be socially isolated or need help with personal care. Services include:

• Transportation
• A hot, noon meal and nutritious snacks
• Personal care and hygiene
• Health promotion and wellness
• Family support and education
• Medication reminders
• Information and referrals
• Supervision and monitoring
• Priority access to St. Ann’s Community Rehabilitation and Skilled Nursing

Activities include:

• Intergenerational programs
• Religious and educational activities
• Supervised cooking and baking
• Individualized Arts & crafts and gardening
• Exercise class and walking club
• Music and sing-alongs
• Field trips and outings
• Cards and games
• Discussion and reminiscence groups

Home & Heart provides daily transportation at a special rate for both ambulatory and wheelchair needs.

All services and activities are included in a daily fee. Customers pay only for the days they attend, must be able to move independently or with a walker, wheelchair or cane and may attend programs from one to five days a week.
