Traditions In Caring is a licensed home health care agency collaboratively owned by St. Ann’s and four other premier long term care facilities. Traditions in Caring provides personalized and individualized services that allow residents to remain in their homes and attain and/or maintain their independence. Services include:

In-home RN case management

• Personal in-home assessments
• Scheduled home visits
• Home health aide supervisions
• Medication pre-pours
• Coordination of in-home personal care

Home Health Aide Service

• Personal care assistance
• Bathing, dressing, grooming and toileting
• Transfers, ambulation and exercises
• Meal preparation
• Medication reminders
• Laundry and light housekeeping

Companion Service

• Accompany individuals to appointments
• Meal preparation
• Laundry and light housekeeping
• Companionship

Care Options

• In-home RN consultation
• Evaluation/assessment of personal needs
• Discussion of health care options
• PRI/Screens for skilled nursing facility placement
