Transitional Care Program

The Transitional Care Program, a unique part of St. Ann 's Community, provides individualized rehabilitation treatment plans by a team of certified professionals who work with you and your family to ensure a smooth transition from the hospital to discharge from our program. This type of plan, when combined with the latest equipment, newest practices and team approach, has yielded amazing results.

Expert Care with a Personal Touch

On-site services in Transitional Care include:

  • Rehabilitation for adults recovering from cardiovascular surgery, amputations, fractures, hip or knee surgery, strokes, swallowing impairments, motor vehicle accidents, neurological and cardiovascular diseases or other acute, complex medical conditions.
  • Physical, occupational and speech therapy utilizing the latest equipment, newest practices and a team approach
  • Clinical assistance with reaching your rehabilitation goals and developing a safe discharge plan.
